USDA Health Travel Certificates

International travel with an animal can be complicated due to the requirements imposed by different countries and airlines.
Many of our veterinarians have the necessary certification to complete USDA paperwork for international travel, however, it is the responsibility of the pet owner to be aware of all airline and country regulations surrounding their travel. Regulations could include specific time frames during which lab tests need to be run or the examinations need to be completed. Please visit the USDA website at for details. Please note, many regulations also apply for travel to Hawaii, but NOT to Puerto Rico.

All travel exams needed to leave the contiguous United States must be scheduled in advance and cannot be completed during our Open Clinic Hours. Please download and complete the following forms. Bring them with you to your scheduled appointment.
